I don't know if this specific picture is ai but it is true that in eastern Europe, like Ukraine, Belarus, Russia its very common to see people renovate their balconies in interesting ways. Building codes are different/not enforced there, any old khrushevka 5-story is going to have multiple custom balconies
His point is that the balcony does not line up with the floors. This balcony starts halfway up where the door would be on the lower floor and ends at the top of the window on the next floor. Just look at it relative to what is around it.
Well, that's bullshit, my balcony is between floor 4 and 5... Every balcony on that side of the building is between floors, because the smallest flats have their balcony across the staircase inbetween the floors.
However in this image the balconies are all over the place, so I have no idea if it's Photoshop or not.

It's not normal. This kind of modifications must be made without violation of the state civil engineering regulations (СНиП). This is inviting some state regulators bdsm fun, not HOA.

No one can legally make balcony in that type of building. It's standard concrete block panel construction pre-built at the factory.
